Inpatient Referral Policy

The Inpatient Rehab Referral Form (available for download below) is to be used for referrals to inpatient rehabilitation at GBGH Midland site.

For each referral, please print and complete the forms listed below and fax directly to GBGH Midland site at (705) 526-0069.

3. Discharge/Transfer Checklist

Sending of updates:  In the event that there is any significant change/deterioration in the patient’s status (ie. Medical, functional, infection status and or equipment needs), notify the team leader at GBGH Midland site at (705) 526-1300 ext 5611.

Discharge/Transfer Checklist:  Upon transfer of patient, please refer to the Discharge/Transfer Checklist regarding the information that is to be sent with the patient.
